Residents of UCSB housing who are unable to use our online Order, Change, or Cancel Service form may also order, change, or cancel telephone, voice mail, calling card, and cable television services using the printable service request forms listed below.

• Service Installation Request
• Service Deactivation Request
• Request for Refund of Charges
• Telephone Services Change-of-Customer Notification (to inform us that you are moving and your roommate wants to take over the telephone)
• Removal of Long Distance Service Request (to restrict your telephone service to Local Only)
• Cable Television Change-of-Customer Notification (to inform us that you are moving and your roommate wants to take over the cable television service)
